Centered on the left side of the label, "SIDE ONE/SIDE TWO" with "SD-5067-A/SD-5067-B" label matrix underneath.  Centered on the right side of the label, "STEREO" with "(AR)" underneath.

"SP-AR" in etched in matrix/(AR) on label indicate Allied Record Company Pressing.

"SP-AR" etching/stamped SRC logo in matrix indicate plating done by Specialty Records Corporation. 

Inner sleeve is card stock with photo that is revealed through die-cut front cover.
